….. Anche nella sezione dedicata all’ipertensione dell’anziano si tratta con attenzione la singola
patologia d’organo ma non si pone alcun rilievo al paziente anziano comorboso o soprattutto fragile.
Valutare l’impatto dell’ipertensione arteriosa nel mantenimento dell’autonomia funzionale nelle attività di base e strumentali della vita quotidiana, o nello sviluppo
della disabilità è invece la grande sfida che si pone di fronte al geriatri in questo campo…...

Since the publication of the last guidelines, evidence from large meta-analyses of published trials confirms that in the elderly antihypertensive treatment is highly beneficial. The proportional benefit in patients aged
more than 65 years is no less than that in younger
patients.

In the elderly, outcome trials have only addressed patients with an entry
SBP at least 160 mmHg, and in no trial in which a benefit was shown achieved SBP averaged less than 140mmHg. Evidence from outcome trialsaddressing lower entry and achieving lower on-treatment values are thus needed, but common sense considerations suggest that also in the elderly drug treatment can be initiated when SBP is higher than 140mmHg, and that SBP can be brought to below 140mmHg, provided treatment is conducted with particular attention to adverse responses, potentially more
frequent in the elderly.

• In elderly hypertensives less than 80 years old with SBP ≥160 mmHg there is solid evidence to recommend reducing SBP to between 150 and 140 mmHg.

to individual tolerability.• In individuals older than 80 years and with initial SBP
≥160 mmHg, it is recommended to reduce SBP to between 150 and 140 mmHg provided they are in good
physical and mental conditions.

In conclusion, targeting a systolic blood pressure of less than 120 mm Hg, as compared with less than 140 mm Hg, in patients at high risk for cardiovascular events but without diabetes resulted in lower rates of fatal and nonfatal major cardiovascular events and death from any cause.However, some adverse events occurred significantly more frequently with the lower target.

……, the 2013 ESH/ESC guidelines state that ‘the HYVET deliberately recruited patients in good physical and mental conditions and excluded ill and frail individuals, who are common among octogenarians, and also excluded patients with clinically relevant orthostatic hypotension’

It is important to remember that both low BP and orthostatic hypotension are associated with syncope, falls and related injuries and fractures (17-19). …………… This population is the one at the highest risk of hypertension-related cardiovascular events, but also of hypotension-related events (19-21). Hypotension-related events are likely to be more common in real life than in clinical trials in which treatment is delivered by expert physicians and patients are followed closely.

Furthermore, interpretation of several aspects of the SPRINT data (lack of beneficial effect on stroke, masking effect of diuretics on signs and symptoms of >3 drug heart failure, BP measuring approach, etc) are still under discussion.28,29 Finally, and more importantly, in patients with advanced frailty, cognitive decline, loss of autonomy and living in nursing home were excluded from the trial.

Exclusion from trial extended to patients with decompensated heart failure, history of stroke, and diabetes mellitus, ie, conditions commonly associated with hypertension in aged individuals in whom they represent a common cause of death. This is a crucial issue also because in SPRINT, the aggressively treated group showed a substantial increase of hypotension, syncope, electrolyte abnormalities, and renal failure, ie, adverse reactions that are likely to be magnified in very old patients, even more so if frail.

2. Valutazione geriatrica: il ruolo del geriatra è quello di approfondire gli aspetti funzionali e cognitivi e di rivalutare la terapia GLOBALE del paziente, andando ad unire le varie competenze specifiche. Attenzione alla rivalutazione di tutti i farmaci e non solo di quelli antiipertensivi (effetti cardiovascolari di farmaci impiegati per altre patologie, ad es. antidepressivi, anticolinergici, neurolettici etc.)

- Problemi legati alla metodica ed ai cut-off nell’anziano che non esistono
- Formazione medici di medicina generale alla lettura corretta del monitoraggio
- Fondamentale nella valutazione del paziente con ipertensione clinostatica ed ipotensione ortostatica (HYP-HYP).
- In presenza di HYP-HYP valutazione della disautonomia:ipotensione ortostatica attiva e passiva; manovra di valsalvae deep breathing

1. Trattamento del paziente con decadimento cognitivo in
atto: verosimile necessità di rivalutare in modo globale il trattamento anti-ipertensivo. Trattare meno? Trattare diversamente?
2. Necessità di studi :- Randomizzati sui diversi livelli di trattamento - Effetti delle diverse classi di farmaci nel paziente con
demenza- End point nell’anziano con decadimento cognitivo e/
nell’oldest old: morbilità e mortalità o qualità della vita?
IPERTENSIONE E DECADIMENTO COGNITIVO
1. Nuovi target di danno: lo studio della funzione vascolare:
- Velocità dell’onda sfigmica- Augmentation index- Pressione centrale e non pressione periferica
2. Ruolo clinico:- Definzione anziani con «successfull aging» da trattare come i giovani- Riproducibili e semplici ma costosi (necessità di apparecchiature più
economiche)
3. Necessità di studi :- Effetti dei vari farmaci su questi parametri- Correlazione tra invecchiamento vascolare e stato funzionale ecognitivo
