Reactions 1462, p29 - 27 Jul 2013 S Risperidone Priapism: case report A 21-year-old man with autism developed priapism while receiving treatment with risperidone. The man had been diagnosed with an autistic disorder at 38 months of age, and he had been receiving risperidone since the age of 10 years. Between 2002 and 2009 he received risperidone 6 mg/day [route not stated]. In late 2009, his dosage was reduced to 3 mg/day. In August 2011, he experienced an episode of priapism for the first time. The man’s risperidone dosage was reduced to 1.5mg qpm; however, he experienced a recurrence of priapism approximately 6 months later. His dosage was reduced to 1mg at night, but he experienced another priapism episode 1 week later. He was then switched from the generic form of risperidone to the brand form, still at 1mg daily. Ketoconazole was also added to his medications. He experienced two more episodes of priapism. Risperidone was reduced to 0.5mg but he experienced a further episode and, in July 2012, risperidone was discontinued. Over the period of one year, he had experienced five episodes of priapism that had required intervention. However, investigations showed that no permanent tissue damage had occurred, and he was not impotent. In August 2012, ketoconazole was discontinued and leuprorelin was started. Leuprorelin was discontinued in October 2012 and, at 4 months’ follow-up, no further episodes of priapism were noted.
