Principali caratteristiche biologiche da ricercare per poter effettuare un trattamento specifico
1. Prendere tessuto adeguato e fare diagnosi di DLBCL, escludendo FL grado 3, BCL-U e Linfoma di Burkitt
2. Se il tessuto è mediastinico, valutare attraverso le caratterstiche istologiche che non si tratti di Linfoma primitivo del mediastino
3. In tutti i casi di DLBCL, determinare se MYC è positivo se fosse possibile con la FISH, altrimenti l’immunoistochimica è un’alternativa
4. Se il MYC è positivo, allora determinare se si tratta di un DOUBLE-HIT tramite la ricerca di BCL2 e BCL 6
5. In tutti i casi, ricercare la cellula di origine usando il gene expression profiling (raramente disponibile e richiede tessuto fresco), un algoritmo in immunoistochimica (di solito la tecnica più usata) o il Lymph2Cx assay (che probabilmente sostituirà l’immunoistochimica nel futuro)
6. Nei casi selezionati, utile testare i seguenti target terapeutici: EBV, CD30, CD19, CD70 e CD22

- leucemia linfatica cronica recidivati e refrattari (Braccio A); - linfomi non Hodgkin recidivati e refrattari (Braccio B).
