Provvedimenti potenzialmente efficaci
1. Cessazione del fumo di sigaretta, evitare utilizzo di stupefacenti (in particolare cocaina)
2. Ridurre la fatica fisica e lo stress psicologico occupazionale
3. Interventi sulla nutrizione: aumentare il consumo di pesce, di acidi grassi ω3 ed evitare
l’iponutrizione
4. Evitare brevi intervalli intergestazionali (6 mesi)
Provvedimenti relativi allo stile di vita ed alla nutrizione
1. Inibizione del travaglio prematuro con tocolisi e somministrazione di glucocorticoidi
2. Prevenzione, diagnosi e trattamento della batteriuria asintomatica e di altre infezioni (es malaria)
3. Cerchiaggio cervicale o pessario cervicale
Trattamenti medici e chirurgici
Ridurre la prevalenza della gemellarita e gravidanza plurima (singolo embrio transfer)
Ottimizzare i trattamenti di ART
1. Ottimizzazione del peso corporeo

Il parto prematuro è il problema centrale dell’ostetricia moderna e richiede misure di screening e di prevenzione
La predizione del parto prematuro grave è possibile con buona sensibilita e specificità
La correzione dello stile di vita, l’adeguamento nutrizionale ed il trattamento medico degli individui a rischio
(farmacologico e/o chirurgico) permettono di ridurre il rischio di parto prematuro di almeno il 50%
Il trattamento con tocolitici, steroidi e magnesio solfato in casi precoci di rischio imminente permette di
abbattere l’incidenza di complicanze e sequele
Il concetto di prematurità va evolvendosi ma la frequenza dell’evento è in lieve progressivo aumento
Il ruolo della ricerca è fondamentale in particolare favorendo collaborazioni multicentriche
