Jesahel, Jesahel, Jesahel, Jesahel.
Mille volti come sabbia nel deserto mille voci come onde in mare aperto
terza strada stan sfiorando i grattacieli quinta strada stanno volando verso il sole.
Jesahel, Jesahel, Jesahel, Jesahel. Liberati dal cemento e dalle luci
il silenzio nelle mani e nelle voci. Jesahel, Jesahel, Jesahel, Jesahel...

● Esordio acuto● Fluttuazioni giornaliere● Non giustificato da nota demenza● Conseguenza di patologia medica generale
INCIDENZA DELIRIUM:
20-40% dei pz. inseriti in programmi CP
66-88% dei pz. in fase terminale
(ultime 48 ore di vita) accusa delirium,

CAUSE potenzialmente reversibili:
● metaboliche: ipercalcemia, uremia, iperglicemia, iperammoniemia
● iatrogene: oppiacei, FANS, BDZ, antidepressivi, neurolettici, anticolinergici (scopolamina, buscopan), steroidi, altri (H2 bloccanti, CT): Forma iperattiva

Le cause più emblematiche
Oppioididisidratazione
fattori (insuff. Renale, disidratazione)

raisal. Cancer 94 (6): 1836-53, 2002.
Delirium ed oppioidi● Dimostrata correlazione positiva
tra insorgenza delirium e livelli ematici M3G M6G
● Prevenzione e/o trattamento: ● 1) stretto monitoraggio sintomi ● 2) riduzione oppioidi ● 3) rotazione oppioidi ● 4) idratazione adeguata ● Lawlor PG, Gagnon B, Mancini IL, et al.: Occurrence, causes, and outcome of delirium in patients with
advanced cancer: a prospective study. Arch Intern Med 160 (6): 786-94, 2000. ]

raisal. Cancer 94 (6): 1836-53, 2002.
Delirium e disidratazione

● Agitazione: in genere evidente (delirium iperattivo)
● Deterioramento cognitivo in genere da ricercare:
● Non ricercato dei medici 30-60%
– Non ricercato dagli infermieri 70%1. Inouye SK, Foreman MD, Mion LC, Katz KH, Cooney LM. Nurses' recognition of delirium and its symptoms:
comparison of nurse and researcher ratings. Arch Intern Med. 2001;161:2467-2473
In pratica
● Medici, infermieri e familiari possono interpretare segni precoci o sfumati di delirium come effetti funzionali di paura, ansia e depressione

Mis-interpretazione da parte dei familiari agitazione = dolore
Armstrong SC, Cozza KL, Watanabe KS The misdiagnosis of delirium. Psychosomatics 38 (5): 433-9, 1997 Sep-Oct
Fick D, Foreman M Consequences of not recognizing delirium superimposed on dementia in hospitalized elderly individuals. J Gerontol Nurs 26 (1): 30-40, 2000
Saper cogliere

●

occurring after delirium. American Journal of Psychiatry, 137, 1433-1435.

1. Breitbart W, Gibson C, Tremblay A The delirium experience: delirium recall and delirium-related distress in hospitalized patients with cancer, their spouses/caregivers, and their nurses. Psychosomatics 43 (3): 183-94, 2002 May-Jun.
●

● Breitbart W,The delirium experience: delirium recall and delirium-related distress in hospitalized patients with cancer, their spouses/caregivers, and their nurses. Psychosomatics. 2002 May-Jun;43(3):183-94
●
burn out● Breitbart W,The delirium experience: delirium recall and delirium-related distress in hospitalized patients with cancer, their
spouses/caregivers, and their nurses. Psychosomatics. 2002 May-Jun;43(3):183-94

riconoscere/accettare i limiti
2) Lavoro e confronto in equipe
● Breitbart W,The delirium experience: delirium recall and delirium-related distress in hospitalized patients with cancer, their spouses/caregivers, and their nurses. Psychosomatics. 2002 May-Jun;43(3):183-94

Efficacia poco documentata, basata sull’esperienza clinica Britton a., Cochrane Review, 2001
● Ingalill R. et al. Impact of delirium on professionals. Dement Ger. Cogn. Desord. 1999; 10: 420-425

1. Meagher DJ, O'Hanlon D, O'Mahony E, et al. The use of environmental strategies and psychotropic medication in the management of delirium. Br J Psychiatry 168 (4): 512-5, 1996. Related Entries

METODI DI GESTIONE NON FARMACOLOGICA
Mantenere ritmi di vita normali (pasti, veglia, sonno)
Favorire comunicazione non verbale (tatto, mimica)
● Accettare critiche dei familiari (meccanismo di difesa)
1. Meagher DJ, O'Hanlon D, O'Mahony E, et al. The use of environmental strategies and psychotropic medication in the management of delirium. Br J Psychiatry 168 (4): 512-5, 1996. Related Entries

Attenzione
● Contenzione fisica raramente necessaria (in genere in ospedale)
1. Meagher DJ, O'Hanlon D, O'Mahony E, et al. The use of environmental strategies and psychotropic medication in the management of delirium. Br J Psychiatry 168 (4): 512-5, 1996. Related Entries

Pensiamoci prima
Prevenzione?
(Hanania M. 2002)
