MAPPET 3 : conclusioni
Treatment with alteplase, given in conjunction with heparin, may improve the clinical course of patients with acute submassive pulmonary embolism and, in particular, that such treatment may prevent further clinical or hemodynamic deterioration requiring the escalation of treatment during the hospital stay.
On the basis of these data, we believe that the indications for thrombolysis,which are currently limited to massive pulmonary embolism, can be extended to include submassive pulmonary embolism (manifested as right ventricular pressure overload and dysfunction) in hemodynamically stable patients.

Considerazioni
2 dei 6 pz con ICH furono sottoposti a TL violando il protocollo per preesistente nota malattia crebrovascolare;
2 eventi si verificarono “tardivamente” (rispettivamente 62 e 157 h dopo TL) e furono probabilmente causati dall’eparina;
La % di ICH non è risultata sostanzialmente dissimile da quella riportata nell’IMA (1.4%)

Considerazioni conclusive
La chiave per una appropriata terapia è la stratificazione del rischio
Pazienti a basso rischio hanno una prognosi eccellente con la sola terapia anticoagulante
Pazienti ad alto rischio (esclusi quelli in shock o con arresto cardiaco d’esordio) possono beneficiare della terapia trombolitica in associazione ad una intensiva anticoagulazione.
