Reactions 1457 - 22 Jun 2013 S Darunavir/ritonavir Acute demyelinating polyneuropathy (first report) in an elderly patient: case report A 65-year-old HIV-infected man developed acute demyelinating polyneuropathy during treatment with darunavir and ritonavir. The man presented with muscle ache and progressive weakness of his lower limbs; he was totally wheelchair-bound. Darunavir 600mg and ritonavir 100mg twice daily [routes not stated] had been added to his antiretroviral backbone regimen of didanosine and abacavir 3 months earlier, in February 2007. Electromyogram results revealed severe sensory peripheral polyneuropathy. All the man’s medication was withdrawn. His clinical state rapidly deteriorated, and his CD4+ cell count fell. He was treated with empirical antiretrovirals including raltegravir, etravirine and emtricitabine/tenofovir disoproxil fumarate. Three months later, his electromyogram results revealed a normal pattern, with no signs of peripheral neuropathy or myopathy. Author comment: "In this report, we present a case of severe HIV-1-related acute demyelinating polyneuropathy which initiated after new generation PI [protease inhibitor] darunavir/ritonavir was given and resolved after cessation of the drug without the use of immune-based therapies . . . The HIV-related inflammatory neuropathies can be further distinguished into a subtype associated with HIV infection per se and an antiretroviral (ARV) toxic neuropathy associated with the use of ARV agents."
