SUMMARY OF MY CLINICAL EXPERINCES

General Medicine Experience

Education: I completed my Medical School in Italy, experience included internal medicine internee and professional training in medical, surgical & gynaecology wards before applying for the State Exam (MRCP equivalent). During my specialisation I received training in Allergology & Clinical Immunology, Cardiology, Geriatrics, Infective Diseases, Acute & Emergency Medicine, Internal Medicine, Sport Medicine, Oncology.

Experience: I have worked at a Consultant level for the Primary Care Trust, Accident and Emergency Unit, Internal Medicine Department. As Consultant in Respiratory Medicine I participate to the Medical Division on call rota for inpatients emergency and medical admission (1:13).

Respiratory Medicine

Education: my post graduate training lasted four years with progressive direct clinical responsibilities. It included clinical responsibilities in Respiratory Ward, Respiratory High Dependence Unit, Intensive Care Unit, Respiratory Medicine and Phthisiology Outpatient clinic, and Respiratory Physiology with excellent experience in practical procedures. Experience: I have worked in different hospitals at Consultant level in Respiratory and Respiratory & General Medicine in the past 16 years. This has given me the chance to lead and develop different services.

Recent Consultant PositionsFrom June 2010 to August 2016 I worked at Pieve di Soligo NHS Trust as Consultant in Respiratory Medicine. In addition to the clinical duties I was the led Physician for Clinical Risk, Diffuse Parenchymal Pulmonary Diseases (multidisciplinary), Prevention & Smoking Cessation Programs. From 01/09/2016 to 08/10/2016 August I worked at Weston Super Mare General Hospital (Weston Area Health NHS Trust) as Consultant in Respiratory Medicine. The position was temporary pending a substantive job, but I was offered a position in Dubai (UAE) that was more attractive in terms of career and service development, in addition to cultural reasons. While at Weston SM GH, my responsibilities included direct care to 12 inpatients (including NIV patients), General Respiratory Medicine (unselected) Outpatient clinic, Pulmonary function tests medical reporting, Specialist Referrals, junior Doctors supervision, contribution towards the improvement of the department (development of Respiratory Services and participation to the local Medical Education program).

Current Consultant Position

From 09/10/2016 up today I work at Valiant Clinic (Meraas Helthcare, Managed by Houston Methodist Healthcare) as Consultant in Pulmonary Medicine.While awaiting for licensing to commence my clinical duties, I am organising the Pulmonary Medicine services that will be provided at the clinic, contributing to the selection of the equipment, writing clinical and procedures protocols, establishing collaboration with local hospitals.At Valian Clinic I will lead the Pulmonary Medicine Department offering a premium outpatient clinic that will include the following services:

* Pulmonary function test- Spirometry- Lung Volumes- Pulmonary Diffusion

* Oxygen assessment- 6 minutes walking test- Overnight oximetry- Arterial Blood Gas analysis- Capillary Blood Gas analysis- Hyperoxic test- Flight assessment

* Sleep Diagnostic- Overnight pulse-oximetry- Cardio-pulmonary polysomnography- CPAP and BiLevel titration

* Respiratory Endoscopy- Diagnostic endoscopy- Bronchial wash- Bronchial biopsy- Bronchial brush- Transbronchial needle biopsy

* Pleural fluid analysis (thoracentesis)

Under my lead Valiant Clinic will be a Philips Respironics Centre of Excellence for Pulmonary Medicine, thus we will provide Medical Education using the most advanced Philips Respironics Technology (provisional agreement submitted)

CLINICAL GOVERNANCE

AUDITI recognise the value and importance of audit in medical practice as a strong tool applied for clinical service

development and improvement. During my Italian clinical experience Clinical Audits were planned if it was

felt there was need for an update on any specific clinical service or before or soon after the introduction of a

new clinical service.

- Diffuse parenchymal lung disease (Pieve di Soligo NHS Trust 2016, ongoing)

This ongoing audit of the first 24 months of sub speciality outpatients clinic on DPLD shows the feasibility

and sustainability of sub speciality clinic in a District Hospital, but also reveals a persistent discrepancy

between expected number of affected patients and patients who seek and receive medical care.

- Smoking habit and long term oxygen therapy (Pieve di Soligo NHS Trust 2014)

My team reviewed smoking habit, depression/anxiety status and caregiver smoking habit of patients on long

term oxygen therapy and found that about 8% of the trust patients are current smokers. This figure reflects

what is reported in the literature. Data analysis showed a link between patient depression/anxiety and care

giver smoking status and persistent smoking habit of patients on long term oxygen therapy

- Smoking cessation clinic for patients with cardio-vascular or respiratory disease (Veneto Region 2012-2013)

This multicenter audit confirmed the feasibility and efficacy of a dedicated service to treat cardiovascular

and respiratory patients for their persistent smoking habit. The audit confirmed a 30% efficacy of the

treatment at 1 year.

- Cardiopulmonary exercise test (CPET) (Bolzano NHS Trust 2005)

This audit revealed that, region wide, more than 100 patients/year could benefit CPET, including borderline

candidates for surgery and patients with exercise induce dyspnoea of unknown origin. As result a CPET

service was funded.

- Outpatient diagnostic investigations and CPAP prescription for OSAS (Bolzano NHS Trust 2004)

This audit evaluated the proportion of outpatient and inpatient diagnostic investigations and CPAP

prescriptions for OSAS. It was concluded that hospital admissions were not required for CPAP prescription

in the large majority of the cases. As result, domiciliary services funds were increased.

- Domiciliary Non Invasive Ventilation (Bolzano NHS Trust 2003)

I participated on a clinical audit that showed that NIV domiciliary services were not uniform throughout the

autonomous province, depending mainly on the prescribing hospital, written carers instructions were not

always complete, and safety controls were not always recorded. As a result, the local NHS authority decided

to regulate NIV prescriptions, centralised NIV to the main regional hospital and standardised domiciliary

requirements.

- Home Oxygen Therapy (Bressanone NHS Trust 2002)

This audit revealed that inconsistent regular assessment of hospital prescription of home oxygen therapy led

unnecessary 30% LTOT prescriptions. As a result it was decided that all Trust home oxygen prescriptions

should have been communicated to the Respiratory Department for authorisation and planning of regular

assessment. LTOT was audited again after the change in the protocol with 0% unnecessary prescriptions.

IMPROVING CLINICAL STANDARDS AND PRACTICE

Interstitial lung diseases (ILDs) Clinic & MDT MeetingTo ensure continuity of care for patients and improve MDT approach to ILDs, I organised a monthly dedicated clinic and local ILD-MDT meeting. This new organisation has been thereafter integrated with a Hub & Spoke regional organisation for IPF therapy prescription. Pieve di Soligo NHS Trust, Vittorio Veneto (TV) 2013-2016

Smoking cessation program for patients with smoke related diseasesA pilot project was launched for active treatment of tobacco dependence in patients with tobacco smoke diseases within the Veneto Region. Interstitial lung diseases (ILDs) MDT MeetingI was asked to organise a regular MDT about ILDs. Despite previous attempts were not successful, stakeholder Consultants were involved in the organisation. Within 3 months Consultants in Respiratory Medicine, Radiology, Rheumatology and Thoracic surgery started meeting regularly and the ILD-MDT meeting became a regular appointment where to discuss ILD cases. Hospital & Domiciliary NIV Service for patients with type II respiratory failureI have been invited to develop and present a business case for NIV in the hospital and domiciliary setting to include the recently approved clinical guidelines for motor neuron disease. Departmental advisory panel for new respiratory physiology equipmentI was invited to lead the panel for the evaluation of the areas of clinical implementation of new non invasive equipment for the determination of cardiac output in respiratory medicine. Provincial advisory committee for the revision of health regulations for NIVI was invited as technical advisor by the local political Health Authorities.
