QUALI LE INDICAZIONI CLINICHE ATTUALI?
EDEMA MACULARE REFRATTARIO:Retinopatia diabetica con edema maculare diffuso o cistoide (CME)
Occlusioni venose retiniche con CMECME in corso di uveiti ed altre indicazioni meno frequenti.
NEOVASI COROID. IN CORSO DI DEG. MAC. LEGATA ALL’ETA’:(in associazione alla PDT) CNV attivi, CNV con RAP, CNV con distacco di

IVT (intravitreale) o ASTA (sottotenoniano)?
IVT è più rapidamente efficace ma anche ASTA è efficace nel medio termine nel DME.
IVT come prima soluzione nei CNVASTA come prima soluzione nel DME (ev. IVT successiva).
QUALI I RISULTATI?
DME: OCT dimostra riduzione dello spessore, più tardivo e meno evidente miglioramento del visus.
A breve termine da diversi studi migliori risultati (OCT e VA) con IVT vs ASTA. Miglioramento a breve termine dei dati PERG. Incerti risultati sul lungo
termine.
CNV: IVT pre-PDT riduce effetti collaterali della PDT, prolunga i suoi effetti vascolari, riduce le complicanze post-PDT.
Rischio: a lungo termine maggiore percentuale di atrofia maculare post trattamento (1).

Ipertono (soprattutto giovani): 48% con 25mg-IVT, 20% con 8mgIVT, 10% con 4mgIVT o 40mgASTA,
normalizzazione a 6 mesi. IVT: Cataratta, Distacco retinico, Endoftalmite (0.2-0.4%)
ASTA: Cataratta, Ascesso orbitario, Perforazione bulbare, Ptosi palpebrale, Atrofia grasso orbitario.
QUALI LE PROCEDURE TECNICHE?
a) 0.1 ml Kenacort senza manipolazionib) 0.1 ml TA-preservative free (tecnica di Jonas)c) 0.1 ml TA-preservative free (tecnica di Tano)
QUALI LE PROCEDURE INIETTIVE?
Linee guida convenzionali (chirurgia bulbare) per la profilassi delle infezioni
ESISTONO PROBLEMI MEDICO-LEGALI?
Sì ! Nonostante la notevole mole di dati clinici disponibili e di trials ufficiali (NEI) in corso
• l’uso del TA (sia IVT che ASTA) è off-label. • Ove possibile, ottenere consenso proprio comitato etico.
• Non procedere solo in caso di palese parere negativo (in corso provvedimenti di organi regionali).

A Study of the VEGF-binding Ability of Aged Bevacizumab
• The usual intraocular dose of bevacizumab for the treatment of AMD is 1.25 mg (0.05 mL of stock). Bevacizumab is supplied in 4-mL (100 mg) and 16-mL (400 mg) preservative-free vials at a concentration of 25 mg/mL.
Preparation of Bevacizumab
• bevacizumab (4 mL: 25 mg/mL). • aseptic technique under a laminar flow hood (ISO Class 5• Puncture of the seal with a Mini-Spike Dispensing Pin• The bevacizumab was then divided into aliquots of
approximately 0.12 mL in multiple 1-mL Luer-Lok syringes. A sterile cap was used to seal each respective syringe.
• Syringes were labeled, placed in light-protective brown plastic bags, and refrigerated at 4°C. One vial of bevacizumab yields approximately 25 x 0.12 mL aliquots
bevacizumab can effectively bind VEGF up to 8 weeks after preparation. Therefore, storage for 2 weeks at 4°C with protection from light seems reasonable. When assaying for sterility, storage for longer periods of time may be a viable option
