Sovradiagnosi (e sovratrattamento)
Sovradiagnosi Almeno 50%

La risposta può venire dai numeri ? O è un problema di altro ordine ?
Significant increment in incidence but stable (or reduced) mortality
Pca carries a life time risk of death: 3%; however, from autopsy series in men > 50 yrs: Pca > 40% (Landis et
al, CA Cancer J Clin, 1999)
One retrospective study (Chodak et al, NEJM,1994) and two prospective studies (Albertsen et al, JAMA, 2006; Johansson et al, JAMA, 2004) suggest localized Pca of low GPS has a low rate of progression to symptomatic or fatal disease within 15-20 yrs from diagnosis
Esiste il rischio di sovratrattamento?
Cooperberg et al, J Urol, 2003
Stage Migration:Low Risk Patients are Increasing
“Indolent disease”: possible future incidence
ERSPC update (C. Bangma et al, Cancer, 2009):“When population-based screening is initiated, it is expected
that approximately 50% of men who are diagnosed with Pca will have indolent cancers…”
Sovradiagnosi = sovratrattamento ?
O
Sovradiagnosi = sovratrattamento ?
Strategy – to decrease overtreatment
• Identify indolent disease
• Consider aggressive and treat non-indolent cases
• Apply active surveillance to indolent cases

Follow-up casi con sorveglianza attiva (PRIAS elegibili ma Gleason 7)Van Den Bergh RCN (PhD thesis, 2009)
casi 50
Sopravvivenza CP specifica a 6 anni 100%
Sopravvivenza globale a 6 anni 68%
Ancora in sorveglianza attiva a 6 anni 59%
Sorveglianza attiva
Raccomandazione: neoplasie cT1c-cT2a. Gleason ≤6.

The problem of opportunistic screening
• Screening may do more harm than good• In the US 70% and in NL 40% of men are PSA
tested• Screening tests cannot be refused to well-
informed men• Balanced, validated information must be
provided

• Considerare sorveglianza attiva !
