• Questi composti sono presenti generalmente come miscele nei prodotti commerciali.

1. Dosis sola facit venenum (Paracelso, 1493-1541)
La tossicità è funzione di: dose x esposizione
Questo principio ha la sua base biologica nel fatto che gli organismi viventi hanno meccanismi di detossificazione o riparazione, a diversi livelli (molecolare, cellulare, tissutale). La tossicità si manifesta quando l’intensità/durata dell’insulto tossico supera la capacità di questi meccanismi
• Non è quindi possibile definire una sostanza come tossica senza definire l’esposizione: dose, durata, frequenza, via di somministrazione.
Variazioni individuali della funzionalità epatica in soggetti che avevano assunto 6 g/die di paracetamolo

Prescott [1983] estimated the threshold acetaminophen dose for hepatotoxicity from a collection of overdose cases.
He estimated the amount of acetaminophen absorbed by multiplying the plasma acetaminophen measured at three hours after ingestion with the volume of distribution (0.8L/kg) and paired the doses with liver damage ratings for all cases.
The results indicate that the threshold dose for hepatotoxicity in humans is approximately 250 mg/kg taken all at once, which corresponds to 12.5 and 17.5 g for a 50 and 70 kg person, respectively.

After a single administration of YIC-C8-434 at 500 mg/kg, female rats had a higher blood concentration of the compound than male rats. Little elimination of YIC-C8-434 was observed in female rats.
Hepatic microsomes from male rats better metabolized YIC-C8-434 than those from females.
Experiments indicated that CYP3A2 played the predominant role in the metabolism of YIC-C8-434 in rats.
Since there is less CYP3A2 in the liver of female than male rats, the involvement of CYP3A2 in YIC-C8-434 metabolism has implications for the sex-related metabolic activity and toxicity of YIC-C8-434.

and colleagues found a trend toward increasing head and facial abnormalities with increasing embryonic alcohol exposure. An effect occurred at even the lowest reported levels of alcohol intake, so that a clear threshold (minimum amount of alcohol to produce an effect) could not be defined.
FAS: influenza di fattori ambientali
• Most reported cases in the United States came from study sites where the mothers were black or Native American and of low socioeconomic status.
• The estimated rate at these sites was 2.6 per 1,000 compared with 0.6 per 1,000 from other study sites, where the mothers were predominantly white and of middle socioeconomic status.
• According to the CDC catchment study, incidences of FAS per 10,000 total births for different ethnic groups were as follows: Asians 0.3, Hispanics 0.8, whites 0.9, blacks 6.0, and Native Americans 29.9.
• Among Native Americans, the incidence of FAS varies among different cultures. Health units serving principally Navajo and Pueblo tribes report an FAS prevalence similar to that for the overall U.S. population, while for Southwest Plains Indians, a much higher prevalence was reported (1 case per 102 live births).
• Several factors, such as cultural influences, patterns of alcohol consumption, nutrition, and metabolic differences have been suggested to play a role in this difference.
• Genetic Susceptibility?• In the case of blacks, the risk of
FAS remains about sevenfold higher than for whites, even after adjustment for the frequency of maternal alcohol intake, occurrence of chronic alcohol problems, and parity (number of children borne).
• This raises the question of some kind of genetic susceptibility, the nature of which is unknown.
Nrf2, which belongs to the basic leucine zipper (bZip) transcription factor family, has been implicated as a key molecule involved in antioxidant-responsive element (ARE)-mediated gene expression.

Effetti acuti e cronici a carico di alcuni sistemi
• Gli effetti dell’esposizione cronica possono manifestarsi come patologie spontanee (es. tumori, diabete ecc.), anche nell’uomo maggiore difficoltà di identificazione dell’effetto.
La tossicità cronica si manifesta a livelli di esposizione più bassi rispetto alla tossicità acuta.
N.B. le dosi che danno tossicità cronica possono dare anche sintomi acuti.
In generale, la tossicità di una determinata dose aumenta all’aumentare della durata e della frequenza dell’esposizione, a causa di due fenomeni:
accumulo del tossico nell’organismo; si ha quando la velocità di somministrazione (frequenza) è maggiore della velocità di eliminazione e la durata è tale che il tossico raggiunge livelli dannosi.

Tossicità ritardata

Generalizations Regarding Toxicologic Information Derived from Organized Clinical Trials
For example, in the early phase I trials of fludarabine monophosphate, the acute delayed toxicity (DLT) in patients with solid tumors was reversible myelosuppression.
The doses subsequently were escalated in patients with refractory forms of leukemia, and a delayed onset of serious neurologic toxicity was observed. Approximately 4 to 6 weeks after the administration of high doses of this agent, cortical blindness and coma developed in the patients.
Therefore, myelosuppression was the DLT in the low dose range, and delayed neurologic toxicity was dose limiting in the high dose range.

• The difference in the delayed onset of a toxic event and that emanating from a cumulative total dose also must be distinguished.
• For example, the neurologic toxicity that resulted from high-dose fludarabine monophosphate was delayed in onset, but it was not simply a function of the total dose. Several patients had received larger total cumulative doses of fludarabine monophosphate administered over a longer time interval, and they did not develop evidence of neurologic toxicity.
• In contrast, cardiac toxicity associated with doxorubicin and pulmonary toxicity produced by bleomycin are examples of toxic events that result from cumulative exposure to the respective drugs.

• In genere, la tossicità sistemica di una sostanza dipende dalla via di somministrazione, cioè dall’entità dell’assorbimento con ciascuna via.
• L’ordine di tossicità è: endovenosa, inalatoria, intraperitoneale, sottocutanea, intramuscolare, orale, cutanea.
• N.B. Molte sostanze possono dare una tossicità locale, con o senza tossicità sistemica
• Via inalatoria: tossicità occupazionale; tossicità di popolazione: inquinanti ambientali dell’aria interna ed esterna.
• Via cutanea: principalmente tossicità occupazionale.
• Via orale: tossicità di popolazione; inquinanti/contaminanti di cibi, acqua ecc. Intossicazioni accidentali o volontarie.
La valutazione del rischio associato ad una sostanza, e le relative decisioni normative, dipendono da:
•quantità e qualità degli studi effettuati
•approccio all’interpretazione e valutazione dei dati degli studi; dipende da fattori socio-culturali ed economici.
