In Italia, ogni anno, si stimano 14.700 casi di CIN 2/3 e 47.000 CIN1
È INDICATO:
PREVENZIONE lesioni genitali precancerose del collo dell’utero, della vulva e della vagina e cancro del collo dell’utero causati da vari tipi oncogenici di Papillomavirus Umano (HPV)
PREVENZIONE lesioni genitali esterne (condilomi acuminati) causate da tipi specifici di HPV.
, NON È INDICATO: (non ha mostrato di avere effetto terapeutico)
TRATTAMENTO del cancro del collo dell’utero, delle lesioni displastiche di alto grado del collo dell’utero, della vulva e della vagina, o dei condilomi genitali.
PREVENIRE LA PROGRESSIONE di altre lesioni già esistenti correlate con il Papillomavirus Umano e non previene lesioni dovute ad un tipo di HPV vaccinale in donne infette dal quel tipo di HPV al momento della vaccinazione.
INFEZIONE NATURALE: meccanismi di ESCAPE con cui HP V riduce la risposta immunitaria
HPV-HR: bassi livelli di proteine virali circolanti, no viremia, non inducono morte cellulare, non determinano infiammazione.

L’ infezione naturale non sempre evoca una risposta immunitaria sierologicamente rilevabile

Objective: to determine whether prior administration of Gardasil can prevent subsequent HPV-related disease after first HPV genital disease
Design: retrospective, ITT population analysis of a subset of women from FUTURE I/II, who had first cervical definitive therapy or VIN, VAIN, GW afterrandomization 1:1 either to Placebo or Gardasil

Objectives and methods (1)
� The aim of the study is to determine if HPV-vaccination coulddecrease the rate of cervical disease recurrence in women affected byCIN undergoing LEEP treatment.
� Patients were prospectively randomized toreceive HPV-vaccination and follow-up(group A) or follow-up alone (group B). Allwomen in group A receive quadrivalentvaccine within 4 weeks post LEEP and theremaining two doses are performed at 2and 6 months after the first.

SperAnZA study: conclusion
� Updated results of SperAnZA study support the utility of HPV-vaccination post-LEEP treatment to reduce recurrence risk.
� Although, results are preliminary, the risk reduction due toquadrivalent HPV-vaccination could be related to lowering risk of newinfections.
