the spores of C. difficile are the transmissible form and contaminate the environment, where they survive for long periods
the ingested spores germinate in the disturbed gut
antibiotics disturb the normal gut flora, some more than others
the C. difficile bacteria produce two principal toxins – A and B –which cause diarrhoea and colitis
the attack rate is variable (greater in older patients), complicatingour understanding of the epidemiology of outbreaks
Pathogenic strains of C difficile produce 2 distinct toxins. Toxin A is an enterotoxin, and toxin B is a cytotoxin. Both are high–molecular weight proteins capable of binding to specific receptors on the intestinal mucosal cells. Receptor-bound toxins gain intracellular entry where they catalyze a specific alteration of Rho proteins, small glutamyl transpeptidase(GTP)–binding proteins that assist in actinpolymerization, cytoskeletal architecture, and cell movement. Both toxin A and toxin B appear to play a role in the pathogenesis of C difficile colitis in humans.
Toxin testing is available seven days a week
Intervals between requests for samples (for hospital in-patients) and their delivery to the laboratory should be minimised
Results are communicated to the ward as soon as they are available
If a commercial C. difficile kit is used, this should have a dual toxin A and B formulation (as toxin A negative/B positive strains exist)
The kit should offer the best performance criteria in terms of sensitivity, specificity and negative and positive predictive values

Only test stools from symptomatic patients, i.e. only liquid/loose stoolsthat take the shape of the container (Bristol Stool Chart types 5–7) should be examined
Do not retest for C. difficile toxin (CDT) positive cases if patients are still symptomatic within a period of 28 days unless symptoms resolveand then recur and there is a need to confirm recurrent CDI
More than one test per patient may be required if the first test isnegative but where there is a strong clinical suspicion of CDI. Retest a second sample 24 hours later
Further tests might be necessary in light of clinical evidence
Generally it is not advisable to test children under the age of 2 years in whom toxigenic strains of C. difficile and toxins A and B may be presentin the absence of symptoms

Sudden increases in the number and/or severity of cases detected in a ward or across several units within a hospital are legitimate reasons fortyping requests
However, this is best undertaken in a planned way, followingdiscussion with the relevant regional laboratory or reference laboratory
Frozen storage of small aliquots of toxin-positive stool samples (e.g. a small Eppendorf tube full at –20°C for a rolling year) is recommended. This is so that a retrospective culture can be made should it becomeapparent that an outbreak of CDI or a change in incidence has takenplace that might warrant culture of the organism for typing
Obtaining isolates is also advisable in order to monitor antimicrobialsusceptibility especially the emergence of resistance to the current first-line treatment options of metronidazole and vancomycin.

Storage temperature and multiple cycles of freezing/thawinghave minimal effects upon the viability of C. difficile or itsspores
Storage at 4ºC has no discernible effect on C. difficile cytotoxin. However, storage at –20ºC has a detrimental effect on the cytotoxin, and multiple cycles of freezing/thawing mayadversely affect toxin titres (Freeman and Wilcox, 2003).

The stool cytotoxin test has high sensitivity andspecificity (ie, 94-100% and 99% respectively)and is the test of choice. Diarrheal stool is filtered and then added to cultured fibroblasts.A positive test result is the demonstration of a cytopathic effect that is neutralized by specific antiserum. This test result is reported only aspositive or negative, it is expensive, and it requires an overnight incubation and a tissue culture facility.

Nei sette (5+2) campioni discordanti è stata eseguita la coltura per l’isolamento del Clostridium difficile e determinazione delle tossine A e B con risultato positivo.
Nei casi in cui si è verificata una discordanza del dato positivo per Ag e tossine o Ag solo,si è provveduto ad una diluizione a raddoppio del campione stesso allo scopo di verificare la soglia di sensibilità del test con cui tale positività è stata evidenziata. Questa sensibilità di rilevazione cambia a seconda del campione in esame essendo correlata alla quantità di Ag e tossina presenti:abbiamo notato che in tutti i casi verificati la quantità di tossina è sempre superiore a quella di Ag ,infatti quest’ultimo risulta irreperibile dal test già dopo basse diluizioni(max1:4),mentre la tossina/e risulta positiva a diluizioni più alte in tutti i casi discordanti verificati.

Gli isolati di Clostridium difficile da coltura sono inviati dal nostro laboratorio all’Istituto Superiore di Sanità (ISS) presso il Dipartimento di Malattie Infettive ,Parassitarie ed Immunomediate(Prof.ssa Paola Mastrantonio)
L’invio tramite richiesta della Direzione Sanitaria si esegue nel sospetto di cluster oppure per valutare la possibile circolazione di un ceppo che può causare infezioni gravi e/o elevato tasso di mortalità. (Es. ribotipo 027)
