Perdite fetali totali*: villocentesi (CVS) transcervicale (TC), transaddominale (TA) e amniocentesi (AC) a confronto

Perdite di liquido amniotico dopo amniocentesi
• Incidenza:1-1,7% entro le prime 48 ore
• Management: conservativo (controllo di liquido, vitalità e crescita fetale)
• Prognosi: generalmente buona– Cessazione della perdita entro 7 giorni
– Ripristino del liquido amniotico entro 3 settimane
– Parto > 34 settimana
– Sopravvivenza perinatale >90%
• Esiti sfavorevoli:– Aborto, morte fetale o parto in epoca previtale
– Perdita cronica con oligo/anidramnios, difetti posturali, ipoplasia polmonare

a second trimester amniocentesis will increase this risk by another 1%.
Early amniocentesis is not a safe early alternative to second trimester amniocentesisOne study compared transabdominal CVS with second trimester amniocentesis and Comparedwith secondtrimesterwith second trimester amniocentesis and
found no significant difference in the total pregnancy lossbetween the two procedures (6.3% versus 7%)
Comparedwith secondtrimesteramniocentesis, transcervical CVS
carries a significantlyhigher riskofpregnancyloss
procedure-related miscarriage rate of 0.5–1.0% for amniocentesis as well
as for chorionic villus sampling
Amniocentesis performed prior to 15 weeks had a significantly higher miscarriage rate than CVS and mid-trimester amniocentesis, and also increased the risk of talipesequinovarus. Amniocentesis should equinovarus. Amniocentesis should therefore not be performed before 15 + 0 weeks’ gestation. CVS on the other hand should not be performed before 10 weeks’ gestation due to a possible increase in risk of limb reduction defects

Precedenti aborti o IVG 8
Women with leiomyomataare at an increased risk for second-trimester spontaneous abortion. Midtrimesteramniocentesis does not appear to further increase this risk.
The incidence of spontaneous abortion was 6.3% among the cases, 0.8% in the amnio-only controls, and 7.0% in the myoma-only controls.
The available evidence suggests the risk of vertical transmission through amniocentesis in vertical transmission through amniocentesis in women with hepatitis B and hepatitis C is not greatly increased. In contrast, the risk of vertical transmission of HIV appears to be increased through amniocentesis. Efforts should be made to avoid inserting the needle through the placenta in all cases.
Results Neither amniocentesis nor chorionic villus sampling was associated with severe pregnancy complications such as placental abruption or placenta praevia. Women in the amniocentesis group had a lower chance of normal delivery (OR ¼ 0.93, 95% CI 0.90–had a lower chance of normal delivery (OR ¼ 0.93, 95% CI 0.90–0.97), an increased risk of complications related to amniotic cavity and membranes (OR ¼ 1.15, 95% CI 1.06–1.24) and hypotonic uterine dysfunction (OR ¼ 1.12, 95% CI 1.06–1.18). The risks were higher for amniocentesis before 15 weeks of gestation. Women in theamniocentesis group were more often delivered by forceps or vacuum extractions (OR ¼ 1.11, 95% CI 1.03–1.19) and elective caesarean sections (OR ¼ 1.09, 95% CI 1.02–1.16). For the chorionic villus sampling group, no significant associations were found.
antibiotic prophylaxis does not reduce the risk of spontaneous reduce the risk of spontaneous
abortion within the 22nd week of gestation
Antibiotic prophylaxis before second-trimester amniocentesis reduced the risk of abortion and of rupture of the
membranes
0,03% vs 0,28%
However, there were several limitations to this trial, such as lack of follow-up until delivery, an unusually high rate of membrane lack of follow-up until delivery, an unusually high rate of membrane rupture within four weeks of the procedure, an unusually high rate of fetal death after membrane rupture (14 percent) in the control group, and the use of a single dedicated operator performing all procedures (raising the issue of reproducibility of the results). Thus, it is unlikely that the conclusions from the trial will change the standard of care, which is not to use antibiotic prophylaxis prior to amniocentesis
