Anemia nella MRC
La gestione dell’anemia secondaria alla malattia renale cronica è un aspetto che richiede la massima
attenzione da parte del Nefrologo rappresentando una abituale e grave complicanza della MRC la
cui frequenza aumenta con il progressivo deteriorarsi della
funzione renale.

Anemia nel nefropatico
La gestione della anemia nei pazienti nefropatici ha subito una vera e propria rivoluzione nel corso degli ultimi venti
anni, dopo l’introduzione nel 1989, della terapia con eritropoietina ricombinante
umana
Un corretto approccio alla terapia dell’anemia associata alla MRC è di
fondamentale importanza per prevenire il rischio cardiovascolare nei pazienti
nefropatici
ESA (Erythropoiesis Stimulating Agents)

L'epcidina è una proteina prodotta fegato che svolge un ruolo chiave nella regolazione dell’omeostasi del Fe
Viene prodotta in quantità definite in funzione: •dei livelli di ferro ematico•dei depositi di Fe •dell'attività eritropoietica
Svolge la sua azione modulatoria legandosi alla Ferroportina inibendo così la liberazione del ferro dalle cellule intestinali e di deposito R.E. al sangue. L’epcidina determina, pertanto, una riduzione dei livelli di ferro extracellulare

Conclusioni Studio CREATE
Sebbene il grado dell’anemia sia un forte fattore predittivo di cattiva
prognosi, la sua completa correzione non determina un miglioramento degli eventi
sfavorevoli.
Lo Studio raccomanda quindi la opportunità di perseguire una
correzione parziale dell’anemia nell’uremico cronico.

METHODS

In this study involving 4038 patients with diabetes, chronic kidney disease, and anemia, we randomly assigned 2012 patients to darbepoetin alfa to achieve a hemoglobin level of approximately 13 g per deciliter and 2026 patients to placebo, with rescue darbepoetin alfa when the hemoglobin level was less than 9.0 g per deciliter
The primary end points were the composite outcomes of death or a cardiovasculr event (nonfatal myocardial infarction, congestive heart failure, stroke, or hospitalization for myocardial ischemia) and of death or end-stage renal disease

CONCLUSIONI DELLO STUDIO TREAT
La somministrazione di Darbepoietina nei pazienti
diabetici, uremici e anemici in trattamento conservativo non
riduce il rischio di entrambi i due outcomes compositi primari
(morte o evento cardiovascolare -morte o evento renale).
E’ stato osservato un aumento del rischio di evento acuto
cerebrovascolare.
SUGGERIMENTI ERBP In patients with type 2 diabetes not
undergoing dialysis (and probably in diabetics at all CKD stages), more caution is needed
when treating anaemia with ESA therapy. In diabetic patients with a history of stroke, a lower target is more sensible (10–12 g/dL), balancing the risk–benefit of treatment and
the desired Hb target in the individual patient. It is also of paramount importance to involve the patient in the decision making, and seek their personal views after a discussion about
the benefits/risks of treatment. On this respect, the patient’s opinion should be
carefully taken into consideration.
The risk–benefit of increased transfusions should also be considered carefully, especially
for patients eligible for transplantation.
SUGGERIMENTI ERBP In diabetic patients with ischaemic heart
disease or with a previous history of stroke, possible benefits of reduced need for
coronary revascularization procedures and transfusions should be weighed up against an
increased risk of stroke recurrence, when deciding which Hb level to aim for, and use of
the lowest possible doses of ESA appears reasonable.
In patients with CKD and a previous history of cancer, the risk of tumour recurrence and related death should be considered when
deciding whether or not to start ESA treatment. Again, in these patients, the
lowest possible doses of ESA should be used.
SUGGERIMENTI ERBP Iron administration is an important factor for the
successful treatment with any kind of ESA, in order to use the lowest dose for reaching and
maintaining the desired Hb target. ESA treatment should not be started in patients
who are iron-deficient. Iron replacement should be used first in any CKD
patient who is proven or likely to be iron-deficient, and only once the iron stores are replete should ESA therapy be initiated.
In CKD patients, ESA treatment should be considered when Hb levels are consistently (i.e. measured twice at least 2 weeks apart) below
11 g/dL (possibly < 10 g/dL in patients with type 2 diabetes and with a history of strokes), and all
other causes of anaemia have been excluded; the threshold for treatment should be decided
according to patient characteristics and symptoms, and the desired Hb target.
SUGGERIMENTI ERBP ESA treatment should be started at a low dose, to
avoid overshooting to high Hb levels; dose adjustments should be made smoothly in the following months in order to avoid too rapid
increases in Hb levels (Hb increases of >2 g/dL per month should be avoided if possible).
The use of high ESA doses in patients who are hyporesponsive to treatment should be carefully
evaluated; increased cardiovascular risk should be weighed against the possible benefits of anaemia correction. It seems wise to avoid progressively
increasing the ESA dose in those patients who do not respond to treatment as expected or in whom it is
obvious that worsening of anaemia is linked to non-renal factors.
The risk–benefit of red cell transfusions should be considered carefully, especially for patients eligible
for transplantation.

ESA therapy should be cautiously used in patients with CKD and
malignancies as no information is available concerning the risk of
mortality and tumour growth in this subset of patients.

Nearly half these patients received darbepoetin alfa (the mean dose was not reported); this cannot be considered true “placebo”
Given that the mean achieved hemoglobin level in the control group (10.6g per deciliter)… there is no evidence that we should stop treating anemia

Il Target di Hb preferibile nei pazienti in trattamento sostitutivo emodialitico in
assenza di cardiopatia severa può essere di 11.0-11.5 g/dl in virtù della mancata evidenza di un beneficio in termini di
sopravvivenza con il ricorso ad un target superiore (Hb = 14.0 g/dl) (livello 2)

Il target di Hb preferibile nei pazienti in trattamento sostitutivo emodialitico con
cardiopatia severa (cardiopatia ischemica: angina pectoris in trattamento, rivascolarizzazione, infarto; scompenso cardiaco con necessità di
ospedalizzazione o ultrafiltrazione) deve essere di 10.0-10.5 g/dl (livello 1)
Target di Hb progressivamente maggiori si associano ad un incremento della qualità di vita,
benchè questo parametro sia stato principalmente valutato in studi osservazionali o con il ricorso a scale di valutazione non validate.

L’efficienza si raggiunge ottimizzando i
parametri delle somministrazioni individuali piuttosto che
applicando uno schema medio a tutti i pazienti.
