Reactions 1456 - 15 Jun 2013 S Omeprazole/trimetazidine Constipation causing valsalva retinopathy: case report A 58-year-old man developed valsalva retinopathy as a result of constipation, while receiving omeprazole and trimeazidine. The man started receiving omeprazole and trimetazidine [dosage and routes not stated] for chest pain. He presented with a 2-day history of central vision loss in his right eye. Fundal examination of his right eye showed a small area of haemorrhage over the supernasal aspect of the macula. He also reported a 2-week history of constipation that had started after initiating trimetazidine and omeprazole [duration of therapy to onset of reaction not stated]. He was diagnosed with valsalva retinopathy; thought to have been caused by straining during bowel motions. The man was managed conservatively. His premacular haemorrhage resolved spontaneously 3 months after his presentation and his visual symptoms subsided. His medication was adjusted and his constipation resolved. Author comment: "We report a case of a gentleman who presented with a 2-day history of sudden loss of right eye central vision and central scotoma. The diagnosis in this case was valsalva retinopathy as a result of constipation, which was a side effect caused by medications (trimetazidine and omeprazole) for his chest pain." Chan JCW, et al. A 58-year-old man developed valsalva retinopathy as aresult of constipation, while receiving omeprazole andtrimeazidine.

The man started receiving omeprazole and trimetazidine[dosage and routes not stated] for chest pain. He presentedwith a 2-day history of central vision loss in his right eye.Fundal examination of his right eye showed a small area ofhaemorrhage over the supernasal aspect of the macula. Healso reported a 2-week history of constipation that had startedafter initiating trimetazidine and omeprazole [duration oftherapy to onset of reaction not stated]. He was diagnosed withvalsalva retinopathy; thought to have been caused by strainingduring bowel motions.

The man was managed conservatively. His premacularhaemorrhage resolved spontaneously 3 months after hispresentation and his visual symptoms subsided. Hismedication was adjusted and his constipation resolved.
