Diabetes mellitus risk factors in rheumatoid arthritis: a systematic review and meta-analysis

• The pooled risk estimate of 11 case-control studies showed a statistically significant increased risk of DM prevalence among RA individuals (OR=1.40, 95% CI: 1.34–1.47).

• The pooled risk estimate of 8 cohort studies also showed a statistically significant increasing risk of DM (RR=1.43, 95%CI: 1.38–1.47).

• In a subgroup analysis for case-control studies, the pooled risk estimate of individuals with RA increased the incidence of T1DM (type 1 diabetes mellitus) and the incidence of T2DM (type 2 diabetes mellitus) (OR, 4.78 vs. 1.41).

• In a subgroup analysis for cohort studies, RA was also found to have a statistically significant increasing risk of T2DM (RR=1.24, 95%CI: 1.14–1.35).

• La diagnosi di diabete mellito indotto da steroidi sulla base della presenzadi due valori a digiuno uguali o superiori a 126 mg comporta la mancataidentificazione di una elevata percentuale di soggetti affetti.

Aggiustamento della terapia insulinica in pazientiprecedentemente trattati con insulina, considerando loschema insulinico abituale e lo schema di terapia con

glucocorticoide

• Se il paziente è già in terapia insulinica si può continuare con la stessadose fino a riconoscere un pattern glicemico specifico che permette di fare adeguati aggiustamenti.

• In alternativa puo` essere utile stimare la dose di insulina da aggiungeresulla base del peso del paziente, della dose e del tipo di steroide e quindidistribuirla nell’arco della giornata a seconda dello schema insulinico in atto.

• Se la dose di glucocorticoidi varia e` necessario aggiustare anche la terapiainsulinica. La riduzione e/o sospensione della terapia con glucocorticoidi in pazienti in terapia insulinica si associa al rischio di sviluppare ipoglicemiase nel contempo non viene ridotta la terapia ipoglicemizzante in atto.

L’effetto iperglicemizzante dei glucocorticoidisomministrati per via intra-articolare, come iltriamcinolone, si osserva gia` dopo 2 ore neipazienti con diabete con un picco d’azione chevaria tra le 2-24 ore e una durata media di circa 2-3 giorni, estendibile fino ai 5-7 giorni.

Conclusioni:Diabete e malattie reumatiche: una relazione complessa e non casuale

• Ci sono elementi comuni nei processi autoimmuni del diabete di tipo 1 e di alcune malattie reumatiche.

• Meccanismi infiammatori comuni nelle malattie reumatiche sono anche coinvolti nella patogenesi del diabete e delle complicanze cardiovascolari.

• La terapia con glucorticoidi è causa più frequente di diabete in pazienti con malattie reumatiche.

• Anche se spesso è necessaria la terapia insulinica, i nuovi farmaci per la terapia del diabete di tipo 2 (GLP1-RA e SGLT2i) che riducono sostanzialmente il rischio cardiovascolare associato alla malattia diabetica hanno una chiara indicazione nei pazienti reumatici.
